    Your account number is indicated on your certificate (directly above your name), dividend cheque or re-invested dividends statement. It is referred to as the "Security Holder Account" on your cheque or statement.

  • Is the online form secure?

Terms and Conditions for Electronic Delivery of Documents

1. I consent, until my consent is revoked by me in accordance with paragraph 8 below, to the electronic delivery of the following documents (the "Documents") of Altus Group Income Fund ("Altus Group") that Altus Group elects to deliver to me electronically rather than by mail, all in accordance with the terms of this Consent:
    - Annual and quarterly reports, including financial statements
    - Other corporate information about Altus Group

2. I understand and agree that, after my consent has been given and Altus Group has filed the Documents with applicable securities regulatory bodies, Altus Group will notify me that a Document that I am entitled to receive electronically is available at the Altus Group website with a link to the specific page of the website containing the Document. I agree that such notification will be sent to me at the e-mail address that I provided.

3. I acknowledge that access to the Internet, e-mail and the worldwide web are required for me to access a Document electronically and I confirm that I have such access and that I have the necessary technical ability and resources to access any such Document.

4. I understand and agree that:
    - Any e-mail notice or other notification will not contain a paper Document.
    - Any e-mail notice or other notification will contain Altus Group's web address (or hyperlink) indentifying where the Documents to be delivered electronically are located.
-    - By entering Altus Group's web address into my web browser, or clicking on the hyperlink, I can access, view, download, and print a paper copy of a Document from my computer.
    - A Document distributed electronically will be in Portable Document Format (PDF).
    - Adobe? Acrobat? Reader software is required to view a Document in PDF format and a link to that software will be available at the page of the Altus Group website containing the Documents to be delivered electronically.

5. I understand that I may request a paper copy of any Document to be delivered to me electronically by either (1) calling Altus Group at 1-800-953-9948, (2) by sending a fax to Altus Group at (905) 953-0586, (3) by sending an email to info@altusgroup.com, or (4) by sending a request in writing by mail to Altus Group Income Fund, Investor Relations Department, 17075 Leslie Street, Unit 7, Newmarket, ON L3Y 8E1, or to AST at the telephone number, fax number, email or mailing address set forth in paragraph 8 below. I acknowledge that my request of a paper copy of any Document does not constitute revocation of this Consent.

6. I understand and agree that:
    - At any time and without giving me advance notice, Altus Group may elect not to send me a Document electronically, in which case a paper copy of the Document will be mailed to me.
    - If a Document intended to be sent to me electronically is not available electronically, a paper copy of the Document will be mailed to me.

7. I understand that Altus Group will maintain on its website any Documents sent to me electronically for a period of time corresponding to the notice period stipulated under applicable legislation and that the Documents will remain posted on the website thereafter for a period of time that is appropriate and relevant, given the nature of the Document.

8. I understand that I may revoke or modify my consent at any time and that I may change my e-mail address to which notices are to be delivered to me at any time by notifying AST by telephone 1-800-387-0825, fax: 416-643-3135, email: http://www.cibcmellon.com/investorinquiry or mail: AST, P.O. Box 7010, Adelaide Street Postal Station, Toronto, Ontario, Canada M5C 2W9. I understand that if I change my e-mail address or revoke or modify my consent, I must notify AST. Such change, revocation or modification must actually be received by AST in order for it to be effective.

9. I understand that I am not required to consent to electronic delivery.

10. I understand that if Altus Group is unable to deliver to my any Document electronically, Altus Group will deliver that Document to me through the mail, together with a notice indicating that Altus Group was unable to deliver the Document to me using the e-mail address provided to it and will provide me with an opportunity to provide Altus Group with a correct e-mail address.

11. I understand that Altus Group maintains in confidence the personal information I provide as a shareholder and uses it only for the purpose of shareholder communication.
