If you would like to take advantage of this electronic delivery program, you must complete the consent form below and return it to the transfer agent of ACE Aviation Holdings Inc., AST by fax at (416) 643-3136, or in the enclosed self-addressed return envelope. ACE Aviation Holdings Inc. encourages you to read the consent form carefully so that you understand the terms under which electronic delivery is made. If you would like to continue receiving these documents by mail, no action is required.

- at any time and without giving me advance notice, ACE Aviation Holdings Inc. may elect not to send me a document electronically, in which case a paper copy of the document will be mailed to me.
- if a document intended to be sent to me electronically is not available electronically, a paper copy of the document will be mailed to me.
7. I understand that ACE Aviation Holdings Inc. will maintain, on its website, any document sent to me electronically, for at least 6 months from the date of posting to the website.
8. I understand that I may revoke or modify my consent and that I may change my e-mail address to which documents are delivered to me, at any time by notifying AST by telephone at 1-800-387-0825, by fax at 416-643-3136, by e-mail at inquiries@cibcmellon.com, or by mail at AST, P.O.
